Use folder prefix is filename is not unique

Summary:
If several files have the same filename in the open document menu,
prefix such with their containing folder name. This changes e.g.
- CMakeLists.txt
- CMakeLists.txt
to
- foo/CMakeLists.txt
- baa/CMakeLists.txt
