Simplify Android build

- pass manifest to add_qt_android_apk, specify name, package name and
  version in it
- move add_qt_android_apk to install phase to be able to build in one
  go
- use the Qt5_android_cmake environment variable to include
  AddQtAndroidApk.cmake to avoid the hard-coded relative location
