Add docbook information to the manpage

The missing date was making ksshaskpass build non reproducible, as the date
gets generated. (http://bugs.debian.org/787115)

So, this change adds the last modified date, and also the other missing fields
that were ending up in visible FIXMEs in the generated manpage.
