Port KDialog to new class DialogBase

That class does not do everything that KDialog used to do, but it
does take care of a lot of the boilerplate code otherwise necessary to
use a QDialog and QDialogButtonBox.  As an added bonus, the sizes of
dialogs are saved and restored when they are next used.
