Add new API for adding/removing child nodes to/from KMime::Content

The main objective of this is to get rid of the implicit multipart
transforms of the old append/remove API. As that needs very careful
porting to find all cases that still rely on the old behavior this
is easiest to ensure with entirely replacing that API.
