Simplify the Add User ID dialog

Reimplement the dialog from scratch
* removing the *.ui file,
* leaving out the comment field,
* leaving out the required labels because marking name and email as
optional is confusing given that one of them must be entered,
* leaving out the error handling for now; accessible error handling will
be added in the next step
