Port away from KDialog and K{H,V}Box

Use natively QDialog and add the missing bits manually.
Initial code thanks to kf5/convert-kdialog.pl, but each QDialog
instance had to be checked and the code adapted to the specific
structure.
Mostly switch to QFormLayout in many places where the structure
matched already its features.
