ExternalToolsPlugin: m_tools should be the central source of all tools

Instead of copying the tools when creating the config page model, use the
tools from KateExternalToolsPlugin::m_tools. More straightforward, easier
to save changes ...etc.

Delay removing tools until apply(), in case the user clicks Cancel in the
parent Kate config dialog.
