Merge Extract actions in toolbar and in File menu

These two actions are basically the same, so it makes sense to merge them into
a single one. This way the CTRL+E shortct is visible in the menu, and no
"Extract" duplicates appear in the Configure Shortcuts/Toolbar dialogs.
