Respect the IndexPolicyAttribute settings for indexing

We now ignore all changes to Collections or to Items beloning
to Collections that have the IndexPolicyAttribute set and
indexing disabled. We also correctly handle (un)indexing of
Items moved from/to Collections with different indexing policies
and when the indexing policy changes.
